This service is for patients that have a diagnosis related to using opioid medication which allows you to obtain a medical cannabis card at a lower price point than the long term cards. REQUIRES FOLLOW UP TO KEEP CARD VALID BEYOND 90 INITIAL 90 DAYS Further information regarding this card can be discussed during the visit.
