To obtain a new or renewal card. Virtual visits will require video capabilities. New refers to any patient that has not been seen by our company before. NON-REFUNDABLE Deposit goes toward final balance. Total price is based upon having a qualifying diagnosis, the $150 is not paid if not qualified. Sales Tax will be applied to final balance.
